Actually 5♣ i mentioned is in respect of the hand i cited in otherwords the squeeze fails(i.e you can't play HA as 9th trick instead you are to cash Cs) if one hand holds both ♥ & ♠ stoppers,in fact the squeeze is independent of distribution only the placement of stoppers are important.In last 5cards whoever holds ♥ stopper can keep only 3♣ & whoever holds ♠ stopper can keep 4♣ but to discard 1♣ on the play of A♥.

OP said defenders failed to take advantage imo i.e by East not discarding a ♣ or playing ♣K at the first two ♣tricks played by South.Without this mistake it can'nt be made.

1.duck the ♥. 2.♥ by West, small from dummy. 3.Cash 3♦, finesse ♣Q,cash ♣A. East is to keep ♥Q & if he does'nt discard ♣K then lead a♣.East can take ♣k &♥Q .Rest is declarer's.If East keeps 2nd ♥ then all♠ are good.

Cash all ♣s & ♦s ending in dummy.Lead ♠10 & let it run . If covered by East by J play Q.

This will be the 6card ending N-NIL-QJ-K10-AX E-Whatever S-XX-NIL-X-KJX W-NIL-AX-QXXX-NIL(or any minor suit combination,what ihave shown is an extreme case) Play, 1.South Spade, discard a Heart from dummy.West can not discard a Heart. 2&3.Diamond to K & ruff a diamond. Same play if West has 4 Clubs instead of 4 diamonds. Club J is important either to take a marked finesse thru' East or to prevent Club 10 if in East's hand to take a trick.
