The word "substituted" is used as an adjective referring to calls or bids on several other occasions in the law book (eg L17D2, L27B1a) and on each occasion it is perfectly clear that the substituted call is not the original call.

There's bound to be a problem with attempting to "punish" players for doing accidentally something that they would be permitted to do deliberately.

Unlikely when we have 98xx over the hearts. And even if partner has HH in hearts we can't lead clubs enough times to stop a heart ruff.
