I think you make 11 in NT unless a club is led as you have time to knock out S's 2 cards then squeeze N in hearts and clubs, although the timing is a bit awkward. If East is choosing Double here (as opposed to 3NT) then they are treating their hand as strong and flexible and should continue with 4♠ over 4♦.
