In an auction that has gone (spades agreed) 4N(blackwood)-5♦(say 1 or 4)-5♥ an explanation of ?Q would be sensible. Typically, the affirmative is to reply as if partner had asked for Kings (or Specific King) and the negative is to return to trumps. But it can be different, for example we play that 5N within a control-bid sequence denies Queen of trumps and skipping 5N to show a control affirms.
