Maybe they play upside down Tho the later underlead suggests not. Btw, the bidding was insane North has an easy heart bid (in my partnerships, we transfer via 2D), and doubling to ‘suggest penalty’ is absurd. Underleading the !H AK twice with south (inserting the 7 or covering the 9) happens to be the only sure way of forcing an entry to the south hand. Forcing the entry to south is only the first step. When south gets in they must lead the spade J and N must NOT ruff. This will eventually allow the defense to score 1c and 1h 3s OR 1c 2h 2s.
