Clearly 3NT is going down with standard signals and normal play at this point, as west will now switch back to diamonds, and five diamonds, 1H and 1S later it is down three. But in general, I would not rule on a hand so early in play, and let both sides have their average minus. The exception necessary for online bridge is is if there was evidence that NS were stalling to prevent the down three from being marked up, in which case I would rule 3NT down three.
