Email Notification of thread messages not working........?

I seem to have things set up correctly in Notification Options/Immediate Notification.

 

Also, I used to receive notifications up to about a year ago.

 

Then they stopped.

 

I've double checked that they're not accidentally being spam filtered.

 

So, any idea what's wrong?
