If partner bids 6♥ I bid 7♣ and over 7♦ I bid 7♥. This hopefully suggests that I do not know whether we should play 7♥ or 7♠. On the given problem, 5♠ No 4d is just what you do when you have opened a wk NT with 3343 or something, and you want to make sure that responder will bid a 5cM if he has one. wk NT opposite 5-4 or even 5-3 in the majors is just so common here that its worth a bid just to transfer the problem back.
