Are you able to know it easily ?I don't say that it is easy, but if North has KQ of clubs and heart fit he should be doing something a bit more imaginative than raising 3H to 4H. Infact jumping on 3♥ meaning heart is selfsupporting and is estabilished as trump suit fourthemore contemporary query for Ace(s) by steps so 3♠(=0 Ace-first step). Now if S bids 4♣ query for King in club in the same way than here 4♦(=0 King) otherwise 4♥(= King of club+almost xx in the trump suit[the convention requires xxx but also xx could be sufficent]). If S want to know about Q/doubleton bids 5♣ and with this information ending in 6♥. The third step indicates singleton (4♠) and raising query suit meaning Ace or void. After 2C-2D-3M, I like this fairly simple continuation method - Any minimum new suit bid shows that Ace. If responder has no Ace but one or more Kings, 3NT is bid.
