I would sign off in 5♠ at this point, because I know slam is at best on a diamond finesse, with additional issues if diamonds are 1-3 (quick ruff for the set) or spades are 4-1 or worse (I don't know the spade jack is in partner's hand). Note that it's worth relaying with 4♥ because I can still play 4♠ when partner has 2 RP, and because if partner has ♦A instead of ♣KQ the slam is virtually cold (in fact makes seven if diamonds divide).
