I just hold a little point that 2D opener has bigger chance for four club than four hearts, and responder has it's chance to bid heart but not likely clubsAnd four cards of club but not five cards of club makes your risk gain ratio increase, thus making a heart lead superior. It wouldn't surprise me to find that partner has four hearts, but I know that I have four clubs.And yes, there might be a stiff Q the dummy but I am still leading small. If I had a diamond trick ion my hand I might give some thought to leading the Q of hearts. The idea would be that maybe partner has five hearts and they have Ax. Or some such. I am not sure I woould do it, but I think that it is worth considering. I would not lead a small heart. On second thought, if I led hearts I would probably lead small. But I doubt I am leading a heart. I did see that the OP suggested that we don't focus on which card to lead. But I don't think that is realistic.
