My scheme using ahydra's suggestion of showing a side suit first rather than shortness :

1NT - 2NT

3H ( showing "good" 4+cd side suit w/ at least 1 Ctrl [ A or K ] AND shortness elsewhere )

I cannot see a way to distinguish the OP hand from ♠AKxxxx ♥KQx ♦xxx ♣x other than by relaying:
