With stronger hand GIB could pass through cuebid overcall or bid game directly. So 2NT is adjusted to 3rd in shown major and 4+ in other major. With 3-3 in majors GIB would bid directly 2NT as invite instead of double. It shows actually 3+ 3+ as minimum is 43 least in random major, but as unclear which is yet is provided like 3+ 3+.So in particular case 2NT will show: Invitational to 3NT game -- 3 ♥; 4+ ♠; 11-12 HCP; stop in ♣- 2NT is adjusted to 11-12HCP as with 13HCP GIB would bid game directly without majors or after if major is not matched. So having this even after 3NT GIB would remain silent as there is no longer chance of hidden fit to be simulated. There is more complicated issue where 4♥ shows 5+♥, but it will be fixed also. With 5 in one major and 11-12 points, not 4+ cds in 2 majors , IMO it should free bid 2s here instead of double. So I think should show exactly 4 spade, not 4+ spade. Also, why should it show 3 of opener's hearts? with say 4=2=4=3/4=2=3=4/4=1=4=4 shape I can imagine bidding double, 2nt.
