So, ok... here is the (my?) answer. Declarer has something like S-A H-Kx D-K9xxx C-ATxxx (maybe diamond more, club less, but I doubt it)... He wants you duck the heart ACE, and he will throw the heart king on the spade king. Win heart ace and exit a spade. You and your partner can wait for the diamond Queen and club king.
