As with many problems, what is the context? Would opener have already bounced with any four-card spade hand? Max with 3? Would M+1 have already shown a max with four? Depending on what NT structure we play (Smolen? some way of showing an invitational 5-5 majors directly?), we probably don't need 3♥ as a natural bid, so that could be a generic game try. Therefore double is not a game try but somewhat penalty-oriented. I would expect opener usually to pass with a doubleton spades and sometimes with three spades, depending on club holding and spade quality.
