I'd normally pass just like Ron, but wouldn't surprise me some people bid 1NT.

(not very serious comment ahead)

There is also the option that at some point I resort my cards and realise I have 5 spades, then I'd bid 4 expect vul vs not, where I'd bid 3

Whatever suits your flavour. In the recent Copenhagen Bridge Invitational tournament, one of the world's top leading pairs of Helgemo-Helness passed their partner's 1H opening with an Ace and a Jack (although with only 2 hearts) and that's just their style.

Playing 2/1 - I have following hand: xx; QJxxx;xxx; Qxx

My partner opens 1. What would you reply?

Have we missorted our hand and do we in fact have 5's or are the Qxx ?

I assume the Qxx are 's. Put me down for 1nt, I will bid 2 over any minor response from p

Pass.

I normally respond with 5 HCP but two queens and a jack outside partner's suit are bad because they may be opposite partner's shortage and so not worth anything. In comparison, a hand like Qx Kxxxx xxx xxx is worth a lot more despite having the same shape and high card points.

We might miss a game if partner has four hearts but even then it will be a thin one. Any time partner doesn't have four hearts we are better off passing.

xx; QJxxx;xxx; Qxx

Depends on how many I have. If I have Qxx then it's a clear 1NT, if I have xx then I guess pass is best, if I have 5 with 2 red suits in between them I guess I'll make a mixed raise.
