Assuming south finds a fairly normal diamond lead, this relies on guessing the spade suit. I suspect that if south does not pass, east should always get this right by playing the heart bidder/takeout doubler for the shorter spades (and the ace). If south passes it seems quite possible to go wrong. I'd expect East to make a further move after 2S (more probable than passing) and E/W reach 4S.
