Suppose East held KQx x AKJTxxx Jx instead of his actual hand. 3NT is completely cold, 4♥ has reasonable chances to go down. ♥Ax support is nearly as good as three of them, and ♠KQ as a stopper sucks. Opener might well have three hearts for 3♦, so West could 3♥ with more or less any five-card suit - xxx K10xxx Jx A10x, for example. After 3♥, East's 3♠ suggests doubt about the right contract. West should clearly bid 4♥ now. I'd blame East. Suppose East held KQx x AKJTxxx Jx instead of his actual hand. 3NT is completely cold, 4♥ has reasonable chances to go down.With that hand, I think East should bid 3NT over 3♥. If 3NT isn't making, what game is? This sort of sequence is made a lot easier by playing 1♦-1♥;2♠ as artificial. But for simple souls like me 3 NT from east did show good stoppers in the black suit and I would be really unhappy after passing this as west with lets say xxx,KQxxxx,Kxx,x. Of course any solution for the bridge world death hand had worked, because then 3 Heart had delivered a 6. heart. And w/o such a tool they had to guess and guessed wrong. Bad luck. Still, with those heart spots, 3N probably isn't right, so West gets the charge if anyone. Good pair of hands for a forcing 2♦ rebid. Using that I think you'd reach 4♥.
