Partner shouldn't have a hand that's 12-14 balanced, so he's either reasonably strong and unbalanced (if he is, his shortness is likely in spades) or 18-19 balanced. In MP 5♦ might be worse than 3NT, but I think there is enough chance of a making slam, or of 3NT being bad , that I will risk supporting ♦ on the 4 level. But as this is the B/I section where unusual vs. unusual is unusual, I would try to raise partner directly. As my hand is too strong for 3 ♦, I would try 4♦.
