In that case, I would expect the first double to suggest a sacrifice rather than be lead directing. The doubler is still going to be on lead when he later doubles. The other possibilty would be that it's a "cancel" on the previously maybe lead directing double. The other option is that this person is also East in the "What do you think partner has" thread. If using Itailian ( or "mixed" cue bids ) then 4D= 1st or 2nd Rnd Ctrl . Opps' 1st DBL shoud be lead directing, but Opener's RDBL says " I have 1st Rnd Ctrl of Diam ".
