I think it's standard to open 1NT by South's hand.

Well, it also seems North's 2 is reasonable with 4423 and 8 HCP.

Obviously South will bid 2

 

Okay now i will try to assume North's options:

A pass

B 3sp - I think then South will decide to 4 with a maximum.

 

Quite surprisingly 4 fails. 3NT succeeds.

 

Any ideas please?

Anything is possible anytime, who knows dear.

As you pointed out to any other suit lead 4 is cold. 3NT doesn't have such a handicap.

If you want a single dummy answer, they are both terrible contracts. If you want an at-the-table answer, they will both make. If you want a double dummy answer then I don't know. If you want an answer in general, south can offer 3NT over 3 after north invites since he is 4333.
