I play it as penalties, but at least partly based on partner's trump holding, not the doubler's. Partner has implied clubs with his take-out double. We are saying that "if you have a normal protective take-out double of hearts I want to defend 2Cx". Invites partner to leave it in for penalties, but he doesn't have to (if he had, say, a 4-2-4-3 10 count). I don't think I'd do it without a heart stop. As Frances says, it's based on some clubs by the protective double. I think he should take it out if he has only two clubs but leave it in with three clubs. I don't think it shows a hand that was going to pass partner's double of 1♥ though - why can't it be a hand with 4 decent hearts, 3 decent clubs and inviting pard to penalty double 2♥?
