"I think I made a major mistake." The hand features a botched auction to 6D. The partnership was at 6C for a minute. That contract actually makes because the Kx in spades is well-placed, allowing that finesse to work and a second entry to dummy's Jxx, AND because the QJxxx is in front of the AK10 of diamonds, AND because Declarer has the Jx of hearts, RHO the KQ, which creates a menace for a throw-in squeeze to enable a double hook of the diamonds without sufficient entries (hence, a "third entry.")
