If you have decided to look for a Heart fit you must also decide not to play in 3NT, (it would be great if partner has 5 Hearts as you can ruff Diamonds in the short hand) so you must bid 4S when Partner bids 3NT it is very likely North has 3 card spade support. In this case the lead coming up to this hand is not a problem. The sequence is only a problem if partner has 2 Spades 4 or fewer Hearts and good stoppers in Diamonds.

Draw trumps ending in dummy. Play DK, DA, Club from Dummy. If East wins (East will have difficulty playing low when holding CQ,) plays another Club on which South discards a Diamond now you have to hope East has no more Diamonds. If East has 8 clubs Only fails if East has a singleton Heart King(90%) or Doubleton HK if holding 7 Clubs over 65%. Unlucky if West wins the second Club but still have other lines mentioned. If East plays a third Diamond you can take the Heart finesse.

Play a Club, West will win play another Club on which South discards a Diamond. East is endplayed.
